CBT for OCD — Taboo Thoughts (ERP)

Overview

Taboo thoughts OCD involves intrusive ideas/images that conflict with one’s values, leading to mental rituals (praying, neutralizing), avoidance, and reassurance seeking.

Why ERP helps

  • Script exposures: write/read triggers verbatim without neutralizing.
  • Response prevention: delay/skip mental rituals and reassurance.
  • Values anchor: act according to values rather than to silence thoughts.

Example exposures

  • Write a brief script of the intrusive thought and read it daily.
  • Look at a triggering word/image while practicing non‑engagement.
  • Attend valued activities likely to trigger thoughts and stay present.

Journaling prompts

  1. Trigger → mental ritual urge → what I did instead → learning.
  2. Peak discomfort __/100; time to halve __ minutes without neutralizing.
  3. Values action I chose while thoughts were present.

When to seek care

Severe distress or complex triggers warrant clinician‑guided ERP and safety planning.
