CBT for OCD — Scrupulosity (ERP)

Educational content only. ERP is best delivered with trained guidance. Work respectfully within the person’s values and traditions. See our Medical Disclaimer.

Overview

Scrupulosity involves obsessions about sin, blasphemy, morality, or violating rules, and compulsions such as confession, excessive prayer, or reassurance seeking.

Why ERP helps

  • Exposure to uncertainty: practice not knowing with humility and values congruence.
  • Response prevention: limit repetitive confession or reassurance rituals.
  • Values alignment: act according to broader values (kindness, integrity) rather than ritual rules.

Example exposures

  • Read a triggering passage without repetitive reassurance or checking interpretations.
  • Write “I might have sinned” and sit with uncertainty while acting kindly in the world.
  • Follow a pre‑agreed limit on confession and practice acceptance of doubt.

Journaling prompts

  1. Trigger → ritual urge → limit I kept → learning about uncertainty.
  2. One values‑consistent action I did instead of a ritual was…
  3. Balanced thought: “Not all doubt requires action; I can live my values.”

See How to Journal for structure.

When to seek care

Complex moral/religious concerns benefit from clinician guidance and, when desired, collaboration with a trusted faith leader.
