CBT for OCD — Relationship OCD (ROCD)

Educational content only. ERP is best delivered with trained guidance. See our Medical Disclaimer.

Overview

ROCD features intrusive doubts about one’s partner or the relationship (e.g., “Do I really love them?”), leading to compulsions like checking feelings, comparing, or seeking reassurance.

Why CBT/ERP helps

  • Uncertainty practice: allow some not‑knowing without resolving it immediately.
  • Ritual reduction: limit checking/comparison and reassurance seeking.
  • Values actions: act kindly and show up in the relationship independent of certainty.

Example exposures

  • Write/read a script: “I might never have perfect certainty about love.”
  • Spend time with partner while postponing reassurance questions.
  • Limit profile/“ex” checking to zero for a day and notice urges pass.

Journaling prompts

  1. Trigger → ritual urge → limit I used → what I learned about uncertainty.
  2. Values action I did today irrespective of certainty was…
  3. How long did discomfort take to fall by half?

See How to Journal for structure.

When to seek care

High distress or relationship conflict benefits from clinician‑guided ERP and communication work.
